Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
Job Highlights
- Visits last about 45 minutes for one-on-one connection and health questions.
- Includes medication and medical history review, physical evaluation, and diagnostic tests (e.g., diabetic eye exams, spirometry).
- Uses an iPad and intuitive clinical workflow.
- No requirement to prescribe medicine, order lab tests, or alter treatment regimens.
Responsibilities
- Bring heart into every visit and improve health outcomes.
- Visit individuals nationwide to provide connections to right care.
- Fulfill the requirements of the specified clinician role.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide Diagnostic & Preventive Services products.
Work Environment
- Primary workplace is the private residence of health plan members.
- Environments are diverse and may contain pets, secondhand smoke, and airborne allergens.
Qualifications
- Active, unrestricted license(s) in coverage area(s).
- Board certification required.
- Multi-state licenses preferred.
