In-Home Nurse Practitioner or Physician Assistant Part Time - St. George
Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to conduct in-home health evaluations and bridge gaps in care. The clinician will perform physical evaluations and medication reviews in members' homes.

Job Highlights
- Conduct 45-minute in-home health evaluations including medication and medical history reviews, physical evaluations, and diagnostic tests.
- Use an iPad and intuitive clinical workflow without the requirement to prescribe medicine, order lab tests, or alter treatment regimens.
- Travel locally and within licensed states as needed, and conduct virtual visits when required.
Responsibilities
- Bring heart into every visit and improve health outcomes.
- Visit members nationwide to connect them to the right care.
- Provide diagnostic and preventive services products such as spirometry, DEE, and bone density screenings.
Qualifications
- Active, unrestricted license in coverage area.
- Board certification.
- Multi-state licenses preferred.
