In-Home Nurse Practitioner or Physician Assistant Full Time - Jersey Shore

Role Overview

• Provide In-Home Health Evaluations in private residences.
• Conduct 45-minute visits including medication review, medical history review, and physical evaluation.
• Perform additional diagnostic tests (e.g., diabetic eye exams, spirometry, bone density screening) if ordered.
• Use an iPad and intuitive clinical workflow.
• No requirement to prescribe medicine, order lab tests, or alter treatment regimens.

Responsibilities

• Build trusted relationships and help bridge gaps in care.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Navigate diverse home environments safely.
