In-Home Nurse Practitioner or Physician Assistant Full Time - Danville-Martinsville

Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations.

Job Highlights

• Conduct 45-minute in-home health visits to connect one-on-one and answer health questions.
• Perform medication and medical history reviews, physical evaluations, and diagnostic tests.
• Use an iPad and intuitive clinical workflow; no requirement to prescribe medicine, order lab tests, or alter treatment regimens.

Responsibilities

• Bring your heart into every visit to improve health outcomes.
• Visit people nationwide to connect them to the right care.
• Work as a clinician colleague fulfilling specified role requirements.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services products including spirometry, diabetic eye exams, and bone density screenings.

Work Environment

• Primary workplace is the private residence of health plan members.
• Must be able to comfortably and safely perform duties in diverse home environments that may contain pets, secondhand smoke, and airborne allergens.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
