In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Providence, RI

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

Job Highlights

• Conduct 45-minute in-home health visits, allowing time to connect and answer questions.
• Perform medication and medical history reviews, physical evaluations, and additional diagnostic tests if ordered.
• Use an iPad and intuitive clinical workflow.
• No requirement to prescribe medicine, order lab tests, or alter current treatment regimens.
• Conduct virtual visits as needed.

Responsibilities

• Bring heart into every visit as part of a national network of clinicians.
• Travel locally and within licensed states as needed.
• Provide Signify Health services including Diagnostic & Preventive Services products.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
