Role Overview
Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations. You will engage with members in their homes to bridge gaps in care and provide comprehensive health evaluations.
Job Highlights
- Visit Details: Approximately 45-minute visits including a medication and medical history review, physical evaluation, and diagnostic tests (such as diabetic eye exams and spirometry) if ordered.
- Workflow: Utilize an iPad and intuitive clinical workflow. No requirement to prescribe medicine, order lab tests, or alter current treatment regimens.
- Responsibilities: Conduct in-home and virtual visits, travel locally within licensed states, and provide diagnostic and preventive services.
Qualifications
- Active, unrestricted license(s) in the coverage area (Hartford, CT)
- Board certification required
- Multi-state licenses preferred
