Role Overview
Signify Health, part of CVS Health, is seeking a part-time Clinician (NP or PA) to provide In-Home Health Evaluations in Barnstable, MA and surrounding areas.
Job Highlights
- Visit Duration: Approximately 45 minutes.
- Responsibilities: Medication and medical history review, physical evaluation, and optional diagnostic tests (diabetic eye exams, spirometry, etc.).
- Workflow: Use an iPad and intuitive clinical workflow; no requirement to prescribe medicine, order lab tests, or alter treatment regimens.
- Travel: Local travel to private residences required; virtual visits as needed.
Requirements
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Ability to perform duties in diverse home environments (may contain pets, smoke, or allergens)
