Role Overview
Signify Health, part of CVS Health, is seeking a full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their homes and helping bridge gaps in care.
Job Highlights
- Conduct 45-minute in-home health evaluations.
- Review medication and medical history, perform physical evaluations, and provide diagnostic tests if ordered.
- Use an iPad and intuitive clinical workflow.
- No requirement to prescribe medicine, order lab tests, or alter treatment regimens.
- Provide diagnostic and preventive services.
Qualifications
- Active, unrestricted license in coverage area.
- Board certification required.
- Multi-state licenses preferred.
