Case Manager - Registered Nurse - Telehealth

Position Summary

• Full-time telework role (Monday-Friday, standard business hours with occasional shifts until 7 PM).
• Specialize in engaging and advocating for vulnerable members with multiple medical, behavioral, and social challenges.
• Assess, plan, implement, and coordinate case management activities.

Key Responsibilities

• Conduct evaluations of members' needs and benefits using clinical tools and data.
• Apply clinical judgment to reduce risk factors and address complex health and social indicators.
• Coordinate clinical resources and collaborate with interdisciplinary team members.
• Utilize motivational interviewing skills to ensure maximum member engagement.

Qualifications

• Unrestricted nursing license with multi-state/compact privileges.
• 3+ years clinical/nursing practice experience.
• 1+ year(s) of medical case management experience, care coordination, or discharge planning.
• 3+ years of experience with personal computers and MS Office Suite.
