Case Manager Registered Nurse (Remote, Texas)

Position Summary

• Acts as a liaison with member/client/family, employer, provider(s), insurance companies, and healthcare personnel.
• Implements and coordinates case management activities for catastrophic cases and chronically ill members/clients.
• Interacts with members/clients telephonically, in person, or via home, worksite, or physician office visits.
• Assesses and analyzes medical and/or vocational status to develop a plan of care optimizing wellness and medical outcomes.
• Prepares required documentation and consults with internal multidisciplinary teams.
• Applies laws, regulations, and special instructions regarding rehabilitation services and insurance carriers.

Required Qualifications

• Active and unrestricted compact RN licensure
• Minimum 3+ years clinical practical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac, home health)
• Minimum 2+ years case management, discharge planning, and/or home health care coordination experience

Education

• Associate degree or Nursing Diploma required
• Bachelor's degree preferred
