Case Manager, Registered Nurse (Remote, New York)
CVS Health is seeking a remote Registered Nurse Case Manager in New York to coordinate care for members with chronic medical, behavioral, and social determinants of health needs.

Role Overview
As a Care Manager Registered Nurse at CVS Health, you will coordinate care for Special Needs Plan (SNP) members facing chronic conditions and social determinants of health challenges. 50-75% of your time is dedicated to telephonic engagement, conducting comprehensive assessments, and developing individualized care plans.
Key Responsibilities
- Compile clinical information and partner with members to develop individualized care plans
- Provide evidence-based disease management education and support
- Ensure interdisciplinary care team involvement and seamless care coordination
- Meticulously document care management activities in electronic health records
- Connect members with health plan benefits and community resources
- Meet regulatory requirements within specified timelines
Qualifications
- Active and unrestricted Registered Nurse (RN) license in New York
- Minimum 3+ years of nursing experience
- Minimum 2+ years of case management, discharge planning, and/or home healthcare coordination experience
- Associate's degree in Nursing (ASN) required; Bachelor's (BSN) preferred
- Proficient in Microsoft Office Suite
