Case Manager Registered Nurse (Remote, East Coast)
Join the CVS Health Community Care team as a remote Case Manager Registered Nurse on the East Coast. Provide telephonic and community-based care coordination to support member wellness and health outcomes.

- Monday - Friday, 8:00 AM - 5:00 PM ET
Position Overview
Community Care Case Managers use a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet an individual's and family's comprehensive health needs.
Fundamental Components
- Act as a liaison with members, families, employers, providers, and insurance companies.
- Implement and coordinate case management activities for catastrophic and chronically ill members.
- Interact with members telephonically, in their homes, worksites, or physician offices.
- Assess medical and/or vocational status and develop care plans to optimize wellness and medical outcomes.
- Prepare all required documentation and interact with internal multidisciplinary teams.
Qualifications
- Active and unrestricted Registered Nurse License in East Coast States
- Minimum 3+ years clinical practical experience with Medicare members
- Minimum 2+ years case management, discharge planning, and/or home health care coordination experience
- Proficiency with standard software and corporate applications
