Case Manager Registered Nurse - Remote

Community Care Case Manager use a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health needs.

Responsibilities

• Acts as a liaison with members, families, employers, providers, and insurance companies.
• Implements and coordinates case management activities for catastrophic and chronically ill members.
• Interacts with members telephonically or in person.
• Assesses and analyzes medical and/or vocational status to develop a plan of care.
• Prepares required documentation and consults with internal multidisciplinary teams.

Required Qualifications

• Active and unrestricted Compact Registered Nurse (RN) Licensure.
• 3+ years of clinical practical experience.
• 2+ years of case management, discharge planning, and/or home health care coordination experience.
• Proficiency with standard corporate software applications.
