Physician Assistant/Nurse Practitioner (ANCC) - Inboxologist - Grand Rapids, MI - Remote - Exp Required

Experienced Physician Assistant or ANCC Accredited Nurse Practitioner to work a remote position managing in basket burden for on-site primary care providers.

• Location: Remote (Candidates cannot reside in CA or NY)
• Schedule: Monday through Friday, 8:00 AM - 5:00 PM

Responsibilities

• Participate in call for second level triage, Monday through Friday 8-5 pm
• Primarily remote position with occasional in person meetings and initial in-person onboarding
• Review patient records, messages, results, and history to assess current health status and develop treatment plans
• Identify real or potential health problems and expected outcomes in response to management plan
• Review patient data with Physicians and APPs for proper treatment
• Collaborate with the care team to provide guidance and outreach to patients and caregivers
• Conduct outreach to providers and staff for follow-up
• Document patient records in the EHR
• Complete billable services such as E-Visits
• Participate in process improvement initiatives for the Inboxology program

Qualifications

• Nurse Practitioner: Master's Degree from an accredited NP program (CCNE or NLNAC), 3 years of relevant clinical experience, and collaborative experience.
• Physician Assistant: Master's Degree, graduate of an accredited PA educational program.
• State Requirements: Active license in the State of Michigan.
• Certifications: BLS, PALS, NRP, and PA-চন্দ্র (for PAs) or ANCC accreditation (for NPs).
