Role Overview
ConcertoCare is seeking an Advanced Practice Provider to be a core member of a multi-disciplinary Field Team delivering home- and community-based care to patients with complex needs.
Responsibilities
- Deliver clinical care flexibly, in-person at the patient's home or virtually from home.
- Provide compassionate, individualized, and holistic patient care including comprehensive primary care, urgent care, and chronic condition management.
- Collaborate with the multidisciplinary care team to deliver transitions of care support, social determinants of health interventions, and complex care management.
- Participate in after-hours on-call coverage as needed.
Requirements
- Nurse Practitioner or Physician Assistant certification.
- Active licensure in the state of Washington.
- Active DEA licensure and state-specific controlled substance registration.
- Certification in basic life support (BLS).
- Valid driver's license with a minimum of 5 years driving experience and a clean driving record.
