Role Overview
Clover Health is looking for a remote, part-time Nurse Practitioner (1099) to conduct comprehensive health assessments and deliver preventive care to Medicare Advantage members using telehealth and proprietary software.
Responsibilities
- Deliver preventive care remotely via software platforms and video apps.
- Complete comprehensive reviews of medical history, physical health, and psycho-social status.
- Document progress notes and encounter summaries using EMR and Clover Assistant.
- Assist at-risk members with transitions from hospitals and facilities to home.
- Create personalized care plans in coordination with primary care physicians.
- Close gaps in care, order labs, write prescriptions, and coordinate follow-ups.
- Perform point-of-care labs and diagnostic testing.
Qualifications
- Must reside and hold an active NP license in Georgia.
- Certified as an Advanced Nurse Practitioner.
- Current unencumbered GA RN license.
- Active DEA and CDS registration.
- Minimum of 1 year clinical work experience providing direct patient care.
- Bachelor’s degree (BSN) from an accredited school of nursing.
- Current Basic Life Support (BLS) provider card.
