About the Role

What You'll Do

• Conduct comprehensive telehealth visits (new patient evaluations, follow-ups, chronic disease management, and medication management).
• Collaborate with care teams, specialists, and patient families.
• Document clinical encounters accurately and timely within the EHR system.
• Identify and escalate urgent or complex cases appropriately.
• Maintain compliance with state-specific telehealth regulations and prescribing guidelines.

What We're Looking For

• Active, unrestricted licensure in the State of New York (required).
• Licensure in additional states is a strong plus.
• Current national board certification as a Nurse Practitioner.
• Minimum 2-3 years of clinical experience, with preference for geriatrics, primary care, internal medicine, or long-term care.
• Comfortable with telemedicine platforms and EHR documentation.
• Strong communication skills for elderly patients and caregivers.
• DEA registration preferred.
• Active malpractice/professional liability insurance (or willingness to obtain).

Compensation

• Average compensation is between $100-$125 per hour with a full-time schedule anticipated.
