ClarityACP is hiring a remote Nurse Practitioner to conduct telehealth advance care planning sessions for patients. This is a 1099 role offering a flexible schedule and an hourly rate of $70.
Job Description
ClarityACP is seeking compassionate Nurse Practitioners to deliver structured, remote advance care planning conversations.
Responsibilities
- Conduct 30–60 minute synchronous telehealth ACP conversations.
- Guide values clarification, surrogate decision-maker designation, and advance directive completion.
- Document visits using EHR templates within 48 hours.
- Accurately record patient locations and follow escalation pathways when necessary.
Qualifications
- Active, unrestricted Nurse Practitioner license.
- Current BLS certification (ACLS preferred).
- Professional liability insurance ($1M/$3M coverage).
- Reliable high-speed internet, computer, webcam, and private workspace.
