Telehealth Emergency Medicine PA/NP – REMOTE
Cityblock Medical Practice is seeking a remote Telehealth Emergency Medicine Nurse Practitioner to provide virtual urgent care and acute clinical management. The role involves working collaboratively on a Mobile Integrated Care team to treat complex patient populations.
Job Description
The Virtual Urgent Care (VUC) Advanced Practice Provider (APP) provides care to Cityblock members as part of our innovative care model. They work on the Mobile Integrated Care team operating 24/7 virtual urgent care and can deploy field teams to members' homes in response to urgent clinical and behavioral needs.
Responsibilities
- Deliver acute care for a complex population of members in a collaborative, interdisciplinary team model.
- Develop treatment plans and address acute needs via phone or video, deploying field teams when necessary.
- Ensure members receive appropriate care for acute and chronic diseases.
- Prevent unnecessary emergency room visits and complete timely documentation.
- Actively participate in Virtual Urgent Care and ED at Home model development and innovation.
Qualifications
- Minimum 3 years of experience as an Emergency Medicine Nurse Practitioner or Physician Assistant in a high-volume emergency department or urgent care.
- Licensed in at least three of the following states: Florida, Illinois, Indiana, Massachusetts, New York, North Carolina, and/or Ohio.
- Experience in acute clinical care, managed care, or value-based care operations.
- Bachelor's degree required.
