Job Description
Circadia Health is looking for a per diem After Hours Tele Nurse Practitioner to cover weekend and evening tele visits for home health patients in the SoCal/OC region.
What You'll Do
- Conduct follow-up and urgent telehealth assessments on flagged patients (5 PM to 11 PM daily).
- Interpret trend-based insights from the Circadia platform and intervene on alerts to prevent ED visits.
- Complete same-day documentation and charts using AI-powered tools.
- Coordinate with on-call providers and care teams.
Must-Have Qualifications
- Certified and licensed Nurse Practitioner in CA.
- ANCC (or equivalent) in Adult, Family, Geriatric, or Acute care.
- 2+ years of NP experience with complex adult populations in home health or post-acute settings.
- Comfortable conducting telehealth visits and navigating virtual care platforms.
