ChenMed LLC is seeking a remote, per diem Nurse Practitioner for weekend and after-hours telehealth coverage to provide virtual primary care consultations and clinical assessments for geriatric patients.

Role Overview
The Advanced Practice Provider (APP), CareLine, PRN is responsible for diagnostic patient care primarily through virtual, remote consultation via video conference or telephone, serving as the dispositional authority for after-hours and weekend clinical calls.
Essential Job Duties
- Provide on-call coverage and assess acute and non-acute clinical problems via virtual video conference or telephone.
- Perform and document physical evaluations, patient histories, and treatment plans based on clinical findings.
- Confer with primary care physicians and other medical providers to report health data and ensure compliance with guidelines.
- Work a minimum of 72 hours per every three months on the weekend, with shifts typically lasting 10-12 hours.
Knowledge, Skills, and Abilities
- Expert knowledge of medical practices to function independently as a certified practitioner.
- Technical capability to conduct telemedicine visits in accordance with state and federal regulations.
- Strong communication, collaboration, and problem-solving skills.
- Proficient in Microsoft Office Suite.
