ChenMed is seeking a remote, part-time Advanced Practice Provider (Nurse Practitioner) to handle virtual consultations and after-hours care primarily on weekends.

Role Overview
The Advanced Practice Provider (APP) I, CareLine is responsible for diagnostic patient care primarily through virtual, remote consultation via video conference or telephone. The incumbent serves as the dispositional authority for after-hours and weekend clinical calls.
Schedule
- Friday meeting: 15:30 - 16:30
- Saturdays: 12:30 - 00:00
- Sundays: 12:30 - 00:00
Essential Duties
- Assess acute and non-acute clinical problems via video or phone.
- Perform and document physical evaluations, patient histories, and treatment plans.
- Coordinate with PCPs, specialists, and case management.
- Maintain regulatory compliance and participate in quality improvement activities.
Qualifications & Requirements
- Multi-state licensure to include FL and at least 3 licenses in VA, GA, MI, MO, OH, PA, TN, TX, IL, or LA.
- Ability and willingness to travel for onsite observations as required by state laws (e.g., Georgia or Tennessee).
- Minimum of 4 years of acute/primary care clinical work experience as a Nurse Practitioner.
