Role Overview
The Advanced Practice Provider (APP) I, CareLine is responsible for diagnostic patient care primarily through virtual, remote consultation via video conference or telephone. The incumbent serves as the dispositional authority for after-hours and weekend clinical calls, accountable for assessing, diagnosing, treating, and documenting patients' physical and psychosocial health status.
Essential Job Duties
- Perform physical evaluations and patient histories via virtual video conference or telephone.
- Analyze trends in patient conditions and develop comprehensive patient management plans.
- Collaborate with primary care physicians (PCPs) and multidisciplinary teams to ensure optimal patient outcomes.
- Adhere to strict departmental goals, regulatory compliance, and telehealth standards.
- Participate in quality improvement activities and clinical team meetings.
Qualifications & Requirements
- Master's degree in Nursing required.
- A minimum of 3 years of acute care, primary care, or urgent care clinical work experience with geriatric patient experience.
- Multi-state licensure including FL, VA, and at least 2 additional state licenses (GA, MI, MO, OH, PA, TN, TX, IL, KY, LA).
- Board certification by AANP or ANCC.
- Basic Life Support (BLS) certification.
- Ability and willingness to travel for periodic onsite observations as required by state laws.
