The Advanced Practice Provider (APP) I, CareLine is responsible for diagnostic patient care primarily through virtual, remote consultation via video conference or telephone. The incumbent serves as the dispositional authority for after-hours and weekend clinical calls.
Schedule
- Friday meeting: 1530-1630
- Saturdays: 0900-2030
- Sundays: 0900-2030
Essential Duties
- Assess acute and non-acute clinical problems via video conference or telephone
- Perform and document physical evaluations and patient histories
- Develop, document, and implement patient management plans
- Collaborate with primary care physicians (PCPs) and multidisciplinary team members
- Utilize telemedicine effectively to ensure optimal patient outcomes
Education and Experience
- Bachelor’s degree in Nursing (BSN) and Master's degree in Nursing required
- Minimum of 3 years of acute or primary care clinical work experience
- Minimum of 2 years of telehealth work experience preferred
