What You’ll Do • Virtual Patient Care: Deliver high‑quality assessments and follow‑up visits via phone, video, or secure chat. • Concurrent Documentation: Accurately type visit notes and update the EHR in real time—maintaining rapport and engagement. • Chronic Care Management: Develop and implement individualized care plans; monitor vitals data from RPM devices; adjust interventions to prevent complications. • Behavioral Health Integration: Screen for depression, anxiety, and other behavioral‑health needs; coordinate warm‑handoffs to in‑house behavioral‑health specialists. • Gap‑in‑Care Identification: Proactively flag and close preventive‑care and disease‑management gaps—especially in patients with multiple comorbidities. • Interdisciplinary Collaboration: Communicate care plans and patient progress to referring physicians, home‑health agencies, and family caregivers. Who You Are • Licensed NP: Active NP license in FL, PA & TX mandatory. • Additional licenses a plus  NY , NJ , OH, GA, MD , NM, or DE . • Telehealth Pro: 2+ years of telehealth experience; comfortable with phone, video, and chat platforms. • EHR Savvy: Proficient with electronic‑health‑record systems and digital-monitoring tools. • Multi‑Tasker: Skilled at simultaneous typing and listening, ensuring accurate documentation without missing a beat. • Communicator: Exceptional verbal and written skills; able to explain complex medical information in a clear, compassionate way. • Clinical Expertise: Background in cardiology, geriatrics, psych/internal medicine, or related fields; strong understanding of chronic‑disease progression and comorbidity interplay. • CCM & BHI Knowledge: Demonstrated experience or certification in chronic‑care management and behavioral‑health integration.
