Registered Nurse – Virtual Care & Care Management (Remote)

CareTalk Health is a virtual medical practice specializing in Clinical Process Outsourcing (CPO). We are seeking experienced Registered Nurses to join our virtual care team in a fully remote capacity.

Responsibilities

Annual Wellness Visit (AWV) Chart Preparation

• Prepare comprehensive AWV charts to support provider efficiency and CMS compliance.
• Review and summarize medical, surgical, and family histories, medication adherence, and preventive screenings with patients via Zoom.
• Identify overdue screenings, risk factors, and care gaps prior to visits.

Care Management & Clinical Oversight

• Provide telephonic and portal-based care management for patients with chronic and complex comorbidities.
• Develop, implement, and monitor individualized care plans (CCM, RPM, RTM, APCM).
• Utilize motivational interviewing to engage patients and caregivers.

Care Navigation & Gap Closure

• Identify and close care gaps regarding screenings, labs, medications, and follow-ups.
• Coordinate orders, referrals, and interdisciplinary care to ensure continuity and quality.

Qualifications

Required

• Active, unrestricted Registered Nurse (RN) license in a compact state.
• Minimum 2–3 years of clinical nursing experience.
• Experience in telehealth, virtual care coordination, CCM, or RPM.
• Proficiency with EHR systems and telehealth platforms.
• Dedicated, private home workspace meeting strict technical requirements (reliable internet, dedicated hardware, wired headphones).

Preferred

• Experience with Medicare Annual Wellness Visits.
• Certification in case management or gerontological nursing.

Schedule & Compensation

• Up to 40 hours per week.
• Core coverage: Monday–Friday, 8:00am–6:00pm EST.
• Weekend coverage required: At least 2 weekend days or 1 Saturday per month.
• Compensation: $35.00 per hour.
