CareGenix Solutions LLC is seeking a part-time remote Texas Nurse Practitioner to act as a Care Manager. In this role, you will conduct virtual patient assessments, coordinate care, and monitor health statuses.
Job Description
We are seeking an experienced and empathetic Texas Nurse Practitioner (NP) to join our team as a Remote Care Manager. Engage with patients virtually to assess, coordinate, and monitor their health care needs.
Key Responsibilities
- Conduct remote patient assessments with empathy and professionalism
- Monitor patients' health status and escalate warning signs appropriately
- Apply medical terminology knowledge to assess and coordinate care
- Communicate clearly and compassionately with patients and families
- Document thoroughly and accurately within multiple computer systems
- Prioritize daily responsibilities while working independently
- Maintain patient confidentiality and HIPAA compliance at all times
Qualifications
- Current, active Texas NP license in good standing
- Minimum 3 years of experience in a clinical or patient-facing setting
- Minimum 3 years of experience in care management or case coordination
- Master’s degree or higher from an accredited school of Nursing
- HIPAA-compliant, private work area at home
