Role Overview
HarmonyCares is looking for a Telehealth Nurse Practitioner (NP) to deliver annual risk assessments in residential or telehealth settings. The provider operates within the scope of practice under a collaborative physician-driven model.
Essential Duties
- Conduct comprehensive health risk assessments for active and chronic conditions.
- Take patient history, perform physical examinations, and determine diagnoses.
- Provide written documentation of patient visits per NCQA standards.
- Take patient vital signs and place case management referrals.
- Communicate with PCPs, caregivers, agency nurses, and vendors.
- Observe OSHA Universal Precautions and maintain patient confidentiality.
Qualifications
- Active/unrestricted nurse practitioner license in coverage states (IA license required).
- Board certification (ANCC, AANP, or NCCPA).
- Active BLS certification.
- Current enrollment in Medicare/Medicaid.
- Valid driver's license and good driving record.
- Outstanding EHR skills.
- Preferred: Geriatric training or experience.
