Responsibilities
- Conduct comprehensive in-home and telehealth health risk assessments
- Take medical history, perform physical examinations, and determine diagnoses
- Provide written documentation of patient visits per NCQA standards
- Place case management referrals and communicate with PCPs
- Maintain patient confidentiality and observe OSHA Universal Precautions
Qualifications
- Active and unrestricted Nurse Practitioner license
- Board certification from ANCC, AANP, or NCCPA
- Active BLS certification
- Outstanding EHR skills
