PRN Nurse Practitioner - Per Visit Telehealth - Remote

Responsibilities

• Conduct comprehensive in-home or telehealth health risk assessments to identify active and chronic conditions, as well as physical, mental, and social needs
• Take medical history, examine patients, and determine diagnoses
• Provide written documentation of patient visits per NCQA standards
• Take patient vital signs and place case management referrals
• Communicate with PCPs, patients, caregivers, and vendors
• Observe OSHA Universal Precautions and maintain patient confidentiality
• Attend required meetings and participate in professional development

Qualifications

• Active/unrestricted nurse practitioner license in coverage states
• Board certification (ANCC, AANP, or NCCPA)
• Active BLS Certification
• Current enrollment in Medicare/Medicaid
• Valid driver’s license and good driving record
• Outstanding EHR skills
• Geriatric training or experience preferred
