Alignment Health is seeking a Field Remote Nurse Practitioner or Physician Assistant to conduct home visits and remote care for senior patients in Ventura County. The role focuses on preventing hospitalizations through in-home assessments, medication reviews, and coordinated care management.
Job Description
Alignment Health is committed to serving seniors and chronically ill patients through innovative care programs. The Care Anywhere (CAW) program provides in-home medical and social support to prevent unnecessary hospitalizations and health complications.
Responsibilities
- Conduct in-home assessments, medication reviews, and health screenings 3.5 days a week and work remotely 1.5 days a week (Monday-Friday schedule).
- Provide patient education and identify diagnoses for active medical management.
- Partner with the Regional Medical Officer to develop appropriate care plans.
- Coordinate care with PCPs, specialists, and ancillary providers.
- Lead a broader clinical team including nurses, health coaches, social workers, and care coordinators.
