Cantex Continuing Care Network is seeking a full-time Telehealth Nurse Practitioner to provide remote clinical care, chronic disease management, and health risk assessments to adult and geriatric members across Texas.

Role Overview
- Position: Nurse Practitioner- Telehealth
- Location: Remote (Texas members)
- Schedule: Full-time
- Reports to: VP Advanced Primary Care Practice
Responsibilities
- Perform virtual comprehensive health risk assessments and video-based evaluations
- Manage chronic diseases and conduct acute symptom triage
- Recommend treatment plans, prescribe medications, and order tests
- Reconcile medications and identify care gaps
- Collaborate remotely with physicians, RNs, and interdisciplinary teams
- Maintain accurate EMR documentation and HIPAA compliance
