Cadence Solutions is seeking a remote Nurse Practitioner to provide proactive, technology-enabled care and chronic disease management for seniors.
Job Description
The Cadence Health team is seeking a remote-based Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey. You will lead clinical decision-making with a strong emphasis on guideline-directed medical therapy.
What You'll Do
- Oversee the monitoring of patient vitals, symptoms, and labs in collaboration with RNs and MDs.
- Manage treatment plans and medications, focusing on heart failure, type 2 diabetes, and hypertension.
- Respond to patient escalations identified via Remote Patient Monitoring (RPM).
- Lead regular virtual check-ins with patients to review labs and therapy optimization.
Requirements
- Active Compact Multi-State RN license.
- Active NP licensure in at least two specified states.
- ANCC or AANP certification.
- 5+ years of experience treating chronic diseases in outpatient or inpatient settings.
- Master’s Degree as a Nurse Practitioner.
