The Role
Cadence is hiring a Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey. You will lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, type 2 diabetes, and hypertension.
Responsibilities
- Manage a panel of patients receiving remote care and make timely clinical decisions based on evidence-based guidelines.
- Initiate and titrate guideline-directed medical therapy (GDMT) in collaboration with physicians and care teams.
- Respond promptly to clinical escalations from remote patient monitoring alerts.
- Provide both synchronous and asynchronous patient care, including virtual visits and secure messaging.
- Meet established productivity, documentation, and quality expectations.
Requirements
- Active Compact Multi-State RN license.
- Active NP licensure in CA, WA, or AZ.
- 5+ years experience treating patients with chronic diseases (CHF, Hypertension, T2D).
- 3+ years of cardiology experience with a focus on heart failure management and GDMT.
- Active ANCC or AANP certification.
- Master's Degree as a Nurse Practitioner.
