Bluestone Physician Services is seeking a part-time Nurse Practitioner or Physician Assistant to provide primary care to patients in group home and residential communities.
Job Description
Role Overview
- Provide on-site, team-oriented primary care to patients in group home and residential communities.
- Manage a panel of patients autonomously, seeing approximately 8-14 patients per day on weekdays.
- Develop and implement tailored care plans in collaboration with clinical assistants, LPNs, RNs, and social workers.
Qualifications
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Valid state professional license.
- Valid driver's license and minimum required auto insurance for local travel.
