Baseline Medical is seeking a remote Nurse Practitioner to provide telehealth assessments and medical oversight for patients receiving in-home care in VA, DC, and MD.

Position Overview
- Provide medical oversight and telehealth assessments for in-home care patients.
- Partner with on-site Registered Nurses during live virtual visits.
- Flexible, availability-based workflow with hourly base pay plus per-assessment compensation.
Responsibilities
- Conduct real-time telehealth evaluations and review RN-collected clinical data.
- Evaluate, diagnose, and manage acute and chronic medical conditions.
- Develop treatment plans, prescribe medications, and order labs.
- Document all encounters accurately in the EMR.
Requirements
- Graduate of an accredited Nurse Practitioner program.
- National board certification (FNP, AGNP, or similar).
- Active, unrestricted NP licensure in VA, DC, and MD.
- Minimum of 2 years of clinical experience.
