Telehealth Family Nurse Practitioner Needed in Louisiana - FT! - $65-$80/hr

Nurse Practitioner – Home Health Assessments Location – Telehealth in Louisiana Start Date: March 2026 Contract Length: 3‑month contract with option to extend Schedule Monday–Friday 8:00 a.m.–5:00 p.m. with a 1‑hour lunch Full‑time only (40 hours/week) Standard daytime schedule No call Schedules released 2–3 weeks in advance Practice Details Home‑based and telehealth assessments within a regional float model Primarily telehealth; occasional in‑home visits if provider lives near patient areas Assessment‑only role; no treatment or prescribing Conduct 5–6 home‑health assessments per day Perform annual risk assessments and chronic condition reviews Escalate urgent findings to the patient’s PCP Paid full day regardless of volume (provider must enter availability as required) 1–3 hours paid orientation; additional modules completed during downtime Provider must be credentialed/enrolled with Medicare Equipment provided and shipped to provider Clinical Scope Complete structured home‑health evaluations (telehealth and in‑person when applicable) Review chronic conditions, capture diagnoses, and update histories Document all encounters according to program guidelines Maintain timely communication regarding escalations Conduct A1C testing when applicable Requirements Nurse Practitioner – Family Practice Board Certified Active Louisiana license required at submission Must be credentialed/enrolled with Medicare Comfortable seeing pediatric and adult patients Prior experience conducting home‑health or telehealth assessments preferred Credentialing Credentialing documents must include active LA license and required certifications Education modules must be completed within 30 days of start Orientation + onboarding fully remote If you are interested or would like to be presented, please reach out to me for more details.
