Aptive Resources is seeking a dedicated and compassionate Registered Nurse Case Manager to support the Teleconology Division at the Veterans Health Services.
Primary Responsibilities
- Coordinate and manage comprehensive patient care plans for veterans
- Collaborate with interdisciplinary teams at the Teleoncology Division
- Conduct clinical assessments and develop individualized care plans
- Monitor patient progress and adjust care plans as necessary
- Educate patients and their families about health conditions and treatment options
- Facilitate communication between patients, families, and healthcare providers
- Maintain accurate and up-to-date patient records
Minimum Qualifications
- Graduation from an accredited school of professional nursing
- Current and unrestricted RN licensure
- BLS certification
- Minimum of 2 years of clinical experience in an acute care setting
- Strong clinical assessment and critical thinking skills
