Job Summary
We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a REMOTE Care Manager (CCM/BHI). This role involves providing comprehensive patient care coordination, assessment, and management through telehealth platforms. The ideal candidate will have extensive experience in acute and post-acute care settings, including critical care, emergency medicine, or hospital environments. The LPN will play a vital role in managing patient health plans, discharge planning, and utilization management to improve health outcomes while ensuring compliance with HIPAA and medical records regulations. This position offers an opportunity to work remotely within a dynamic healthcare team dedicated to delivering exceptional patient service and care.
Duties
• Conduct thorough patient assessments via telehealth, including but not limited vital signs and symptom review
• Develop and implement individualized care plans based on clinical evaluation and patient needs
• Coordinate discharge planning, post-acute care, and outpatient services to ensure seamless transitions of care
• Manage utilization review and case management activities to optimize resource use and patient outcomes
• Document all patient interactions accurately within medical records, adhering to HIPAA guidelines
• Collaborate with multidisciplinary teams including physicians, behavioral health specialists, and social workers
• Provide education on disease management, medication adherence, and lifestyle modifications
• Monitor patients' progress through diagnostic evaluation results and follow-up assessments
• Assist with intake procedures for new patients requiring managed or primary care services
• Support hospital medicine, Geriatrics, Behavioral health, Infectious disease care, and other specialized areas as needed
Experience
• Proven experience in critical care environments.
• Familiarity with inpatient and outpatient clinic operations along with post-acute care management
• Knowledge of physiology, medical terminology, diagnostic evaluation processes, and discharge planning procedures
• Prior involvement in utilization management, case management, or patient service roles in hospital or clinic settings
• Strong understanding of HIPAA regulations related to medical records and patient confidentiality
• Ability to perform comprehensive patient assessments including vital signs and symptom analysis
• Experience working with diverse populations such as geriatrics, or behavioral health patients is advantageous
• This position requires a dedicated healthcare professional committed to delivering compassionate patient-centered care remotely while maintaining high standards of clinical excellence.
