Remote On Call Nurse Practitioner or Physician Assistant Certified***PRN***Multi State Licensure**

Role Overview

Altea Healthcare is looking for a Certified Nurse Practitioner (NP) or Physician Assistant (PA-C) for a remote, part-time, night-shift position in post-acute care.

Responsibilities

• Take remote calls for Chronic Care Management (CCM) visits from 6 PM to 6 AM on a rotating schedule.
• Examine patient medical records.
• Order and interpret diagnostic tests.
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and create treatment plans.
• Collaborate with physicians, nurses, and facility staff.
• Educate patients and families on health conditions and preventive care.
• Document patient care accurately and efficiently.

Requirements

• Active PA-C or NP license in Illinois and Ohio required prior to start date.
• Willingness to obtain licensure in Indiana.
• Current and active DEA registration preferred.
• Background or passion in acute care, family health, geriatrics, internal medicine, or primary care.
