Remote On Call Nurse Practitioner or Physician Assistant Certified***Multi State Licensures***

Role Overview

• Location: Remote (Illinois, Ohio, and Indiana)
• Compensation: $80,000 - $110,000 per year
• Job Type: Full-Time
• Shift: Night Shift (7pm - 7am, rotating)

Responsibilities

• Take remote calls for national-level CCM visits during scheduled night shifts, holidays, and evenings.
• Examine patient medical records and oversee overall health.
• Order and interpret diagnostic tests (labs, imaging, etc.).
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and create treatment plans.
• Collaborate with physicians, nurses, and facility staff.
• Educate patients and families on health conditions and preventive care.
• Document patient care accurately and efficiently.

Qualifications

• Active NP or PA-C license in Illinois and Ohio required prior to start date.
• Willingness to obtain licensure in Indiana.
• Experience in acute care, family health, or geriatrics is preferred.
• Strong communication, problem-solving, and patient-first approach.
