Role Overview

- Location: Remote (Must reside in Michigan)
- Compensation: $80,000 - $110,000 per year
- Job Type: Full-time
- Shift: Night shift (8 PM - 8 AM), rotating weekdays, evenings, and holidays
What You'll Do
- Take calls remotely for national-level Chronic Care Management (CCM) visits
- Examine patient medical records and oversee overall health
- Order and interpret diagnostic tests (labs, imaging, etc.)
- Diagnose and manage acute and chronic conditions
- Prescribe medications and create treatment plans
- Collaborate with physicians, nurses, and facility staff
- Educate patients and families on health conditions and preventive care
- Document patient care accurately and efficiently
Who Should Apply
- Certified Nurse Practitioner (NP) or Physician Assistant (PA-C) license or eligibility to obtain
- Must be licensed in Michigan
- New graduates and experienced providers welcome
- Experience in acute care, family health, geriatrics, internal medicine, or primary care
- Strong communication, problem-solving, and patient-first approach
