Job Description
Alignment Health is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to manage complex patients in a collaborative, home-based setting.
General Duties
- Monday-Friday schedule with no call and no weekend requirements.
- Conduct in-home assessments 3.5 days per week, with 1.5 days remote.
- Perform comprehensive history, physical exams, medication reviews, and safety screenings.
- Handle end-of-life care discussions and laboratory specimen collection in the home setting.
- Identify diagnoses for active medical and chronic disease management.
- Communicate findings to Primary Care Providers and provide patient/family education.
Minimum Requirements
- At least 1 year of prior clinical experience.
- Computer literacy and EMR proficiency (Epic experience preferred).
- Active state license as a Nurse Practitioner or Physician Assistant.
- Active NP/PA Board Certification.
- Valid driver's license and automobile insurance.
