Alignment Health serves seniors and the chronically ill, aiming to transform and save lives. The Virtual Care Center (VCC) provides 24/7 telehealth services to prevent unnecessary hospitalizations and health complications.
Responsibilities
- Conduct virtual visits and comprehensive assessments (past medical history, symptom review, physical exam, medication review, cognitive/depression screenings).
- Provide patient education on chronic conditions, self-care, sick-day plans, and medication compliance.
- Identify diagnoses and formulate a list of current and past medical conditions.
- Communicate assessment findings to generate post-visit summary notes for PCPs and identify gaps in care.
- Participate in weekly interdisciplinary team meetings to develop care plans.
- Comply with all HIPAA regulations.
Job Requirements
- Experience: At least 1 year of prior clinical experience (3 years preferred; geriatric experience preferred).
- Education: Master's degree from an accredited NP or PA program.
- Skills: Knowledge of clinical standards of care, independent work capability, strong communication, problem-solving, and organizational skills.
- Schedule: Flexible schedule including alternative shifts (day, evening, overnight, weekends, holidays).
