San Diego County Field Remote Nurse Practitioner or Physician Assistant (Must be able to conduct home visits in San Diego County)

Alignment Health is looking for an Advanced Practice Clinician for the Care Anywhere (CAW) program to provide medical and social support to seniors in their homes, aiming to prevent unnecessary hospitalizations and health complications.

Responsibilities

• Conduct in-home assessments, medication reviews, and health screenings.
• Provide patient education aligned with best care practices.
• Identify diagnoses for care management and active medical management.
• Partner with the Regional Medical Officer and team members to develop care plans.
• Coordinate care with PCPs, specialists, and ancillary providers.
• Lead the broader clinical team including nurses, health coaches, social workers, and care coordinators.

Requirements

• Education: Master's degree from an accredited NP or PA program.
• Experience: None required; 1 year of clinical or home care experience preferred.
• Skills: Excellent communication, problem-solving, organizational, and time management skills.
