Alignment Health is seeking a Nurse Practitioner or Physician Assistant to conduct in-home assessments and provide clinical care to seniors and chronically ill patients through their Care Anywhere program.

Role Overview
Alignment Health is looking for an Advanced Practice Clinician for the Care Anywhere (CAW) program to provide medical and social support to patients in their homes, preventing unnecessary hospitalizations and managing chronic conditions.
Responsibilities
- Conduct in-home assessments and occasional outpatient clinical work.
- Perform comprehensive history, physical exams, medication reviews, and screenings.
- Engage in end-of-life care discussions and laboratory specimen collection.
- Identify diagnoses for active medical management and chronic disease care.
- Communicate assessment findings and care gaps to PCPs.
- Educate patients, families, and caretakers while connecting them with community resources.
- Comply with all HIPAA regulations.
Job Requirements
- Experience: No prior experience required; 1 year of clinical or home care experience preferred.
- Education: Master’s degree from an accredited NP or PA program.
- Schedule: Monday-Friday, 8am to 5pm, with a 4/10 schedule option.
