About the Position
Alignment Health is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to manage complex patients in a collaborative, highly coordinated home-based setting. Work closely with physicians, APPs, social workers, case managers, and nutritionists.
Responsibilities
- Conduct in-home assessments (max 5 per day) and outpatient clinical work
- Perform comprehensive history, physical exams, medication reviews, and cognitive/depression/safety screenings
- Handle end-of-life care discussions
- Collect laboratory specimens in the home setting when appropriate
- Identify diagnoses for active medical management and chronic disease management
- Communicate findings to PCPs to close care gaps
- Provide patient, family, and caretaker education
- Comply with HIPAA regulations
Requirements
- Computer literacy and EMR proficiency
- At least one (1) year prior clinical experience
- Active California state license as an NP or PA
- Active NP/PA Board Certification
- Medicare Part D Prescriber
- Valid California driver license and current automobile insurance
- Knowledge of current clinical standards of care
- Ability to work independently
