Alignment Health is seeking a Field Remote Nurse Practitioner or Physician Assistant to support the Care Anywhere (CAW) program, delivering in-home care for our members.
Key Responsibilities
- Conduct member assessments
- Review medications
- Prepare care plans in collaboration with the care team to prevent hospitalizations
Qualifications
- Active NP/PA license
- Master’s degree
- Preferred experience in geriatric or home care
