Field Remote Nurse Practitioner/ Physician Assistant (hospice/palliative care experience preferred)

About the Position

Alignment Health provides care and support to seniors and chronically ill patients in their homes through the Care Anywhere program. The role focuses on preventing unnecessary hospitalizations and managing disease progression.

Responsibilities

• Conduct in-home assessments, medication reviews, and health screenings for members.
• Provide patient education aligned with best care practices.
• Identify diagnoses for care management and active medical management.
• Partner with the Regional Medical Officer and team members to develop appropriate care plans.
• Coordinate care with PCPs, specialists, and ancillary providers.
• Lead a broader clinical team including nurses, health coaches, social workers, and care coordinators.

Requirements

• Master's degree from an accredited NP or PA program.
• Ability to communicate effectively and solve problems in a fast-paced environment.
• Active Nurse Practitioner or Physician Assistant license.
• Active RN license and furnishing number (NP only, upon hire).
• Active Nurse Practitioner Board Certification or Physician Assistant Certification.
• NPI Number, DEA, and valid BLS.
• Valid driver's license and current automobile insurance.

Nice-to-Haves

• Hospice/palliative care experience preferred.
• One (1) year of prior clinical or home care experience.
• Previous EMR experience.
• Experience caring for older adult (geriatric) patients.
