Virtual Primary Care Nurse Practitioner / Physician Assistant

Advocate Health is seeking a full-time Advanced Practice Registered Nurse or Physician Assistant for their GCMG Virtual Care department to provide comprehensive virtual care.

Responsibilities

• Assess, diagnose, and determine treatment plans via virtual health modalities.
• Order, perform, and interpret diagnostic studies and prescribe interventions.
• Manage conditions based on evidence-based care and risk/benefit assessments.
• Leverage digital diagnostics and remote physiologic monitoring.
• Provide health promotion, disease prevention, and patient education.
• Maintain accurate documentation in the electronic medical record.
• Facilitate coordinated care and communication across the healthcare team.
• Participate in quality, safety, and peer review initiatives.

Requirements

• Active RN, APRN, or PA license in state of practice.
• Active national board certification (Nurse Practitioner or NCCPA).
• Active DEA registration.
• Master's or Doctorate's degree for APRN; Bachelor's, Master's, or Doctorate's degree for PA.
• Typically requires 3 years of experience.
• High level of clinical proficiency and decision-making skills.
